Reference Screening Form
Your name has been given to us as a reference for an individual that is seeking to work with children or youth at the Plymouth Alliance Church. Kindly give us your opinion of the individual in terms of character, appearance, personality, personal data, etc. Our desire is to provide a safe atmosphere that enables our children, youth or special need adults to learn and experience the love of Christ as represented in our volunteer staff. Your reply is kept in strict confidence and is not seen by the applicant. Please give us your estimate of the individual's fitness in each of the items following.
